How to Apply – Tier 3 Housing

We are accepting applications for Tier 3 transitional housing.

Eligibility Criteria

Referral from a community organization that provides services to unhoused adults.
The candidate must:
- Be employed in a tax-paying job (day labor is not eligible). Employment will be verified.
- Be sober.
The candidate agrees to:
- Random UA (urinalysis) testing.
- Engage regularly with an Advance Pathways case manager.
- Meet with other guests to share their story of movement toward self-sufficiency.
- Follow all behavior rules.
- Contribute 30 % of monthly income toward program fees.

Applications are accepted in person when meeting with a staff member or over the phone: 970-299-5960 (ask for the Tier 3 Resident Program).

Monday–Friday from 9 a.m.–2 p.m.
The program staff member will conduct a brief confidential screening and perform a background check.
You will be provided a call-back time to receive the status of your application.
